Immunizations

  • All students entering school for the first time must show proof of immunization or a medical objection to the immunization. The parent(s) must show proof of immunization on the first day of school and not less than 20 days after enrollment. If proof is not shown within this time period, the student is excluded from school and the immunizations must be obtained within a certain time frame as requested by the administration.

    The State of Indiana requires the following immunizations for school entrance:

    Pre-K

    3 Hepatitis B
    4 DTAP (Diptheria, Tetanus, & Pertussis)
    3 Polio
    1 Varicella (Chickenpox)
    1 MMR (Measles, Mumps, & Rubella)
    2 Hepatitis A

    K-5th GRADE

    3 Hepatitis B
    5 DTAP
    4 Polio
    2 Varicella
    2 MMR
    2 Hepatitis A

    6th-11th GRADE

    3 Hepatitis B
    5 DTAP
    4 Polio
    2 Varicella
    2 MMR
    2 Hepatitis A
    1 MCV4 (Meningococcal)
    1 TDAP (Tetanus, Diptheria, & Pertussis)

    12th Grade

    3 Hepatitis B

    *3rd - 5th grade students may obtain the 2nd MMR any time desired before entering the 6th grade. TB skin tests are optional, but encouraged at kindergarten and 6th grade entrance along with a physical examination.

    Clinics for immunizations are held at each elementary building in the spring to update the MMR and Tetanus vaccinations for incoming 6th graders. A parent/grandparent must accompany the student to the clinic. Please watch for information regarding the dates of the clinics.

    A clinic will be held for graduating seniors in May. Each student will be given the opportunity to receive a Tetanus booster and a TB skin test. Help will also be given in filling out health records for college entrance. Parent permission is required for students under 17 years of age. Please watch for information regarding the date of this clinic.

    Vaccines are listed under routinely recommended ages. Bars indicate a range of recommended ages for immunization. Any dose not given at the recommended age should be given as a "catch-up" immunization at any subsequent visit when indicated and feasible. Ovals indicate vaccines to be given if previous recommended doses were missed or given earlier than the recommended minimum age.
